Discussed with Director concerns regarding flying fruit flies that were found during this inspection.
Today an unannounced monitoring visit was conducted with the Director. This visit took place in the morning between 9:05-12 there were 24 children with 4 staff within the supervision guidelines. The children were observed learning numbers, counting, singing and dancing to songs, lunch and in transition for nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, medications and emergency procedures. Standard #: 22VAC40-185-270-A Description: Based on observations made, it was determined that areas and equipment in the center were not maintained in a clean, safe and operable condition. Evidence: 1. Chipped/peeling paint were found along the water fountain in the school age classroom, bathrooms, bathroom partitions, and in the 3's classroom along the wall near the water fountain and along the bookshelves/cubbies. 2. a hole the size of a quarter was found along the wall near the parent board and cordless phone. Plan of Correction: Director stated that she will have those areas re-painted by the end of the month and will have the shelves covered.
Standard #: 22VAC40-185-510-A Description: Based on records reviewed, it was determined that 1 of 3 long-term medications did not include required documentation. Evidence: A long-term medication for Child #1, did not include a parent or a physician written authorization. Plan of Correction: Director stated that she will have the parent complete a new authorization form by the end of the week.
Standard #: 22VAC40-185-550-M Description: Based on records reviewed, it was determined that a random sample of written injury reports did not include documentation of required information. Evidence: 7 reports did not include date and time parents were notified and documentation on how parent were notified. Plan of Correction: Director stated that she will make sure that staff are filling out the form in its entirety.
