Comments:
An unannounced, mandated monitoring inspection was conducted today. Twenty-four children were in care with three staff when the inspector arrived at 7:40 AM. The inspector departed at 11:00 AM. Morning arrivals, activities, snack service and preparation for field trip departure were observed. A sample size of five staff records and five children's records were reviewed. Medication administration and injury records were reviewed. An exit meeting was conducted with the program director prior to departure for the field trip.

Standard #: 22VAC40-185-380-A-2
Description: Based upon observation and staff interview, the facility has not ensured that rest mats or cots are used for nap time for preschool aged children. Evidence: 1. The facility has one preschool aged child (child 5) in care for all day care (spring break) this week. Although the facility has a designated quiet time each day, the facility does not have a cot or rest mat for use by the preschool child during rest time. 2. Staff 1 verified that child 5, although five years old, is still in preschool, not kindergarten, and that the facility currently did not have rest mats.

Plan of Correction: The facility responded with the following: Rest mats will be purchased for use when any preschool children are in care for the whole day (such as spring break camp). Sheets/blankets will be used with the mats.

Description: Based upon review of five staff records, the facility has not ensured that they obtain the results of a criminal history/fingerprint record check before three years since the date of the most recent criminal history/fingerprint check. Evidence: 1. The most recent criminal history record check for staff 2 is dated 4/2/2015. A more recent criminal history/fingerprint record check was not in the record provided. 2, Staff 1 verified that, although a fingerprint check has been scheduled for staff 2, the results have not yet been obtained.

Plan of Correction: The facility responded with the following: A fingerprint screening has already been scheduled for staff 2. Due to a backlog of screenings by the fingerprint agency, a screening could not be scheduled any earlier. Results will be placed in the staff records as soon as they are received.
