Technical Assistance:
We discussed the requirements for children's records, age/identity verification, emergency drills and annual staff training. The risk assessments for violations were provided. I measured the proposed downstairs child care room and viewed the placement of equipment and supplies.

Thank you for your assistance during today's unannounced monitoring inspection conducted from 1:05 PM to 5:10 PM. Today, there were seven children, ages seventeen months through five years, in attendance during the morning with two staff. Four children remained for lunch. There were not any children present this afternoon. I viewed equipment, staffing, posted information, daily schedule, snack menu, emergency supplies and drill records, six records for children, three staff records, agency reports, and indoor areas. We talked about diapering procedures. There was a variety of age appropriate equipment and supplies for children. Active play has been in the gym during the cold weather season. Standard #: 22VAC40-185-140-A
Description: Based on a review of six records for children, and an interview with administrative staff, there was not a physical examination record for child 1 which was required no later than the first month of attendance. Evidence: The first date of attendance was July 24, 2017 and there was only an immunization report in the file.

Plan of Correction: The child care manager will call the parent and request a copy of a physical examination record to be submitted when child 1 returns on a scheduled day this week. In the future, the child care manager will use a system for tracking record needs and reminding parents of record needs. As needed, parents are told the information is required prior to the next attendance.

Standard #: 22VAC40-185-40-G
Description: Based on a review of six records for children, and interviews with staff, the center failed to view and document proof of identity and age for child 6. (Repeat violation.) Evidence: Child began attending in August 2017 and attended five dates in August and some dates in each month since. However, the section for staff to complete the viewing of a birth record was left blank and there was not a copy of a birth record or documentation of a report to local law enforcement when proof of age and identity was not verified within seven business days of initial attendance.

Plan of Correction: The child care manager will contact the parent again and ask to view a copy of a birth record. If a copy of the birth record is not brought in within seven business days, the child care manager will follow the requirement for reporting to a local agency.

Standard #: 22VAC40-185-240-C
Description: Based on a review of three records for staff, and an interview with administrative staff, staff member 3 obtained less than sixteen hours of annual staff development activities for 2017. Evidence: There were training certificates totaling fourteen hours of training for 2017. Administrative staff did not have any additional documentation available for staff member 3.

Plan of Correction: The child care manager will plan to conduct reminders to staff about training hour requirements so that part-time workers understand training needs. Also, staff will be reminded to use down time with low or no attendance to take online courses. Staff will participate in a minimum of sixteen hours of training each year.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the evacuation drill records, and an interview with administrative staff, there was not a monthly practice drill conducted in March 2018. Evidence: The last evacuation practice date in the record was February 28, 2018. An administrative staff person did not have information for a March drill.

Plan of Correction: The child care director will use a reminder system for conducting monthly evacuation practice drills so that each are conducted as required.
