22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:33am-1:30pm. At the time of entrance, 85 children were in care with 17 staff members present. Children were observed during snack time, reading books, free play, dancing, singing, art activities, infant care and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center, inside and outside, were not maintained in a safe and operable condition. Evidence: 1. In one of the toddler classrooms, a soft blue climbing structure had material that was coming apart or ripped and had a large opening. 2. In a toddler classroom and in the two year old classroom, the mini play kitchen had a missing part which caused a hole in the equipment that could cause a finger entrapment. 3. In the school age classroom, the paint is chipping under the board and on the wall near the wood blocks. 4. On the playground, a mini slide had a large crack in it. Plan of Correction: The climbing structure was thrown away and will be replaced next month. The holes in the mini kitchen will be filled. The mini slide was thrown away. The school age classroom will be repainted.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center did not cover all unused electrical outlets when not in use. Evidence: 1. In the Toddler classroom, one high level outlet was not covered near the radio. Plan of Correction: It was covered immediately.
Standard #: 22VAC40-185-520-C Description: Based on review, the center is not following all requirements when diaper ointment or cream is used. Evidence: 1. Child 1 and Child 2 had diaper ointments or creams with the child's name on it, but the center did not have written parent authorization. Plan of Correction: Creams will be filled out today, until the forms are filled out.
