Discussed snack procedures, and children's records.
Today an unannounced monitoring inspection was conducted between the hours of 2:30pm and 4:45pm. I observed 15 children directly supervised by 3 staff. Ratios and supervision were in compliance. Children were observed arriving to the program, doing homework, eating snack, coloring, playing table games and participating in group play. Today's snack consisted of Cheerios and milk. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Medication management was in compliance. Standard #: 22VAC40-185-140-A Description: Based on a review of three children's records, Child A and B were missing physical examination forms. Evidence: Child A with an enrollment date of 08/14/2017, and Child B with an enrollment date of 3/05/18 were missing physical examinations from their records. All physical examinations shall be completed within 30 days of the child's enrollment. Plan of Correction: We will request a physical form from their parents.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the center did not maintain complete children's records. Evidence: Child A, C, and D are missing the parents' place of employment information, and their workplace telephone numbers. Plan of Correction: I will add the information to our registration form.
Standard #: 22VAC40-185-540-A Description: Based on observation, the first aid kit was incomplete. Evidence: The first aid kit was missing a second triangular bandage. Plan of Correction: I will pick one up from the office.
Standard #: 22VAC40-185-560-F Description: Based on observation, the snack menu for the current week was not posted. Evidence: Last month's snack menu was posted, the snack menu for the month of April was not posted. Plan of Correction: I will get one from the office on Monday.
