VENDSUB-000 SPECIAL SERVICES
The licensing inspector completed a Subsidy Health and Safety inspection in response to a self-report concerning dietary restriction that occurred at the center on 03/01/2018. The Department was notified of the incident on 03/01/2018. The inspector conducted a monitoring inspection on 03/26/2018 from approximately 10:00AM until 1:50PM and interviewed staff, made observations at the center, and reviewed documentation. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self-reported concern is determined to be valid. See the violation notice for more details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: VENDSUB-000-033-F Description: Based on staff interviews and documentation review, staff served a food to a child that was listed as a dietary restriction.
During the inspection, the licensing inspector interviewed staff and reviewed documentation and the following was determined:
1. Child #1 is listed on the Allergy/ Food Preference form as not to be served meat. In the Discovery Preschool classroom on 03/01/18, Child #1 was served a piece of turkey for afternoon snack by Staff #1 in the Preschool classroom. Staff #2 saw that Child #1 had taken one bite of the turkey and removed the turkey from the plate. Staff #2 stated Staff #1 was not typically assigned to the classroom.
Plan of Correction: The employee that served meat to the child received a written warning for not flowing policy when serving food on the day of the occurrence. This warning is permanent part of the employee?s file. In addition, the next day, he received one on one training from the Director of how to properly utilize the Allergy/Preference Meal Packet. This training was documented and placed in his training file. For the 2 weeks following the incident the Director shadowed the employee at snack time to ensure that he was following proper procedure. The Director continues to do spot checks with the employee to make sure standards are being maintained. These checks are documented in a spreadsheet by the Director. The entire staff was given training on the proper use of the Allergy/Preference Packed the week following the occurrence, and it is documented in the Employee Training Binder. In addition, the Director does random spot checks to ensure that all staff members are referencing the Allergy/Preference form at each meal time. All spot checks are documented on a spreadsheet by the Director. The Director meets with the cook each day to determine what alternatives will be served to children with food preferences of food allergies to the menu foods of the day. The alternative is then prepared and clearly marked with the name of the child to which it is to be served. All meeting are documented in a spreadsheet by the Director. The 2 weeks directly following the incident, the Director emailed the parent of the child and told them exactly what he had to ear for breakfast, lunch and afternoon snack. At the end of the 2 weeks that father informed us that he had regained confidence in our care standards and that the emails were no longer deemed necessary.
