Comments:
An unannounced monitoring inspection was conducted on April 11, 2018. At the time of entrance there were 28 children present with 5 staff members. The sample size consisted of 9 children's records and the emergency information and health related information for 5 staff. Children were observed playing outside, during story time and preparing for lunch. Three medications were observed. Previous violations were reviewed. The inspection began at approximately 9:25 am and ended at approximately 12:05 pm. Areas of noncompliance are noted on the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review of nine records and staff interviews, the licensee did not ensure a separate record for each child was kept at the center and included all required information for each child enrolled. Evidence: During the inspection on April 11, 2018, the following records were determined to be incomplete; 1) The records for child #1, #6, #8 and #9 did not include proof of identity 2) The records for child #3, #4 and #7 did not include previous childcare.

Plan of Correction: The center director had the records updated for child #1,#6 and #8 during the inspection. The other records will be updated today,

Standard #: 22VAC40-185-90--A
Description: Based on a record review of 9 records and staff interviews, the licensee did not ensure the required three written agreements between the parent and the center was in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent. Evidence: During the inspection on April 11, 2018, the records for child #1 and #6 do not include the three parental agreements listed below; 1) An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds;The record for child #1 2)A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center; 3)A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.

Plan of Correction: The center director had the record for child #6 updated during the inspection and will have the parent of child #1 update the record today.
