Comments:
An inspection was conducted and completed after the center called and reported a concern. The Inspector observed classes, interviewed staff, reviewed records, and checked special care procedures. The Inspector observed diaper changes in two classes and talked to one staff about procedures. The classes were in ratio and staff were able to answer the Inspectors questions related to allergies, medication, and recording incidents. Please call with questions 703-479-4679

Standard #: 22VAC40-185-60-A
Description: Based on record review of children's files, the center does not have complete records for Child 1 and 2. Evidence: 1. Child #1 has two emergency contacts listed on the enrollment form, but one emergency contact does not contain the address. 2. Child #2 has two emergency contacts but neither indicate where they are located.

Plan of Correction: Have the parent update emergency contact address.

Standard #: 22VAC40-185-340-A
Description: Based on staff interviews. observation, and record review, it was determined the staff have not ensured the care and protection of children in the center. Evidence: 1. Interviews with staff revealed children in the three's class and one child in the four's class have been hurt by one child. 2. Review of incident and accident reports revealed three children have been bitten and hit with objects by the same child and 5 of these incidents took place within 10 days. 3. Accident reports documented that child #4 was hit and bitten by the same child two times within one week.

Plan of Correction: Closely observe in monitor, however the child's last day will be on 4/27/2018

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff did not wash their hands after each diaper change in the two's class. Evidence: 1. On April 10, the Inspector was observing the program in the two's class and staff #1was changing diapers. 2. Staff #1 had finished a diaper change and called another child to climb up the steps so her diaper could be changed. Staff #1 did not wash her hands between the two diaper changes.

Plan of Correction: Review diapering procedure with the staff member.
