Comments:
An unannounced monitoring inspection was completed on 4/16/18. There were 106 children present in seven groupings with 16 staff directly supervising, a kitchen staff person, and two administrative staff to assist with supervision as necessary. Ten children's records and five staff records were reviewed. Four additional staff records were reviewed to observe background checks only as they were newly hired staff. Two over-the-counter, as needed medications were reviewed for two infants. The inspector arrived at the center at 9:00 am and departed at 2:15 pm. The children were observed during free play times in the classrooms, during movie times, during preparation for lunch, during lunch, and during nap. The school-age children were present due to schools being closed because of the inclement weather/natural disaster after-effects and their activities to include lunch and movie time were observed. There was discussion with the director about the following: the sanitizing solution used and the method of following the manufacturer's instructions for drying time, repairs needed to the walls in the cafeteria, background check changes.

Description: Based on observation and interview, the center failed to ensure that hazardous substances that were in substitute containers clearly indicated the contents. Evidence: 1. There were generic spray bottles observed in two classrooms. One of the bottles was labeled "sanitizer" and did not clearly indicate the contents. Another bottle had been labeled previously and did not have a label on it clearly indicating the contents. 2. There was a commercial substance in a spray bottle that was labeled with the original manufacturer's label. According to the staff in the classroom, the contents was not the original substance. The staff stated that this spray bottle was re-filled with a different substance that is pre-mixed and commercially purchased by the center. The bottle did not clearly indicate the substance in the spray bottle.

Plan of Correction: This was corrected during the inspection. All of the bottles were labeled during the inspection to clearly indicate the contents.
