A Subsidy Health and Safety monitoring inspection was conducted on 4/18/2018 to review supplemental health and safety requirements. This inspection began at 9:25am and ended at 1:45pm. There were 9 staff with 41 children in 2 classrooms at the time of the visit. At the start of the inspection, children were observed playing independently and going outside to play. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were no medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Standard #: VENDSUB-000-006-B-11 Description: Based on record review, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy. Evidence: Child #1, Child #6 and Child #8 have been diagnosed with a food allergy and have emergency medication on site. There are not allergy care plans from the children's physician's instructing staff on steps to be taken in the event of a suspected allergic reaction. Plan of Correction: We will reach out to the parents to get those forms completed.
Standard #: VENDSUB-000-012-B Description: Based on record review, all staff members have not completed the Virginia Preservice Training. Evidence: Staff #1 did not have documentation of completed Virginia Preservice Training. Plan of Correction: This staff member said that she will bring in a copy tomorrow.
Standard #: VENDSUB-000-012-F Description: Based on record review, all staff members do not have current CPR and first aid training. Evidence: Staff #1 and Staff #3 do not have documentation of current CPR and first aid training. Plan of Correction: Staff #1 has completed it and we need to find the certificates. Staff #3 is in the process of getting their training.
