An investigation of a self-reported incident regarding sight and sound supervision was conducted on April 11, 2018 from 11:20 a.m.-12:20 p.m. Based on three staff interviews and director interview, there was evidence to substantiate the incident. Description: Based on three staff interviews and director interview, the center did not ensure that all children under 10 years of age were within actual sight and sound supervision. Evidence: On Thursday, March 22, 2018, Child A, age two, was left alone in the Orange/Yellow classrooms' bathroom by Staff A for approximately 10 minutes while the staff and other children were in the dining car eating lunch.

Plan of Correction: An accountability training was conducted for all staff. All classrooms now have two clipboards so that both the lead and assistant teacher in each classroom can verify the attendance at all times. Staff A was immediately terminated. A meeting was held with the parents of Child A. The director reported the incident to licensing.
