An unannounced monitoring inspection was conducted today with the director, who was present with a second staff and 21 children. The children were returning from washing their hands and then were given their afternoon snack ( cereal and milk). Afterward, some of the children washed their hands and then tranistioned to homework/zone time. There were no medication on site and there was only one injury/accident report to review. Three staff and seven staff files were reviewed today. Arrival time: 4: 05 pm Departure time: 5: 40 pm
Standard #: 22VAC40-185-160-A Description: Based on a review of staff files and an interview with the director, it was determined that one staff did not have adequte documentation on file of a negative tuberculosis screening/x-ray/test. Evidence Staff A was hired on 9/11/17, the chest x-ray on file was dated 6/21/11. Documentation of a negative tuberculosis test/screening/x-ray must been no later than 21 days after employment or within 12 months prior to employment. Plan of Correction: Staff A will obtain an up to date x-ray/screening.
Standard #: 22VAC40-185-70-A Description: Based on a review of three staff files and an interview with the director, it was determined that there was missing information from one staff person's file. Evidence: There was no written information to demonstrate that the Staff A possesses the education,training, staff development, certification, and/or experience required by the job position. Plan of Correction: I will talk with Corporate office about obtaining the required information.
Standard #: 22VAC40-185-500-A Description: Based on observation , it was determined that children did not wash their hands with soap and water or disposable wipes after eating snack. Evidence: The Licensing Inspector observed 7 children finish their afternoon snack and failed to wash their hands with soap and water or disposable wipes. Plan of Correction: We will be mindful to have those children who finish snack later to wash their hands with soap and water or to give them disposable wipes to clean their hands.
