- Technical Assistance:
Discussed with center management improvising staffing/programming for the 2 year old classroom, background checks.
An unannounced monitoring inspection was conducted from 10:20 am-2:30pm. 68 children were in care with 12 staff members present. Children were observed engaged in free play, using table top and floor toys, centers including home living, building blocks, writing. There was no outdoor play time due to rainy weather. 5 staff, 7 children records and emergency drill records were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-130-B Description: Based on record review, center did not obtain additional immunization records once every six months for children under the age of two. Evidence: Child # 1's record contained the most recent immunization records from July 2017. Additional immunization records should have been obtained in January 2018. Plan of Correction: I have asked the parents and will remind them.
Standard #: 22VAC40-185-350-F Description: Based on observation and record review, center did not obtain written parental permission to assign children to different age groups. Evidence: 1. 4 children were moved out of the Preschool classroom and assigned to the Pre-K classroom for transition purposes. 2. There were no written parental permissions found in the children's records to allow the center to assign these children to a different classroom. 3. Staff had documented on the Child Supervision Records the times when the children were arriving and leaving the classrooms. Plan of Correction: I have verbally spoken to the parents and will obtain written permission.
Standard #: 22VAC40-185-550-M Description: Based on record review, the written records for minor injuries did not include all the required information. Evidence: 1. Six incident reports were reviewed. 2. Two of the six incident reports were missing the time that the parent was notified about the minor injuries. Plan of Correction: I will review the accurate method of completing the incident reports with all staff.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and staff interview, center did not obtain the results of a central registry finding for staff within 30 days of employment. Evidence: 1. Staff # 1 was hired on 2/21/18. Her record did not have a central registry finding. 2. Staff # 2 stated that she had submitted the central registry after Staff # 1 was hired. 3. Staff #1 stated that she received a phone call from the agency informing her that some information on Staff # 1's form was unclear. Staff # 1 stated that the agency requested the corrected information over the phone. 4. The phone call was received approximately four weeks prior to this inspection. Center did not make any efforts to follow up since then. Plan of Correction: Center management was trying to contact the agency during the inspection. I will try to follow up on the status of the form.
