Comments:
A Subsidy Health and Safety inspection was conducted on 04/23/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from 11:00am-3:30pm. There were 68 children (infants-5yrs.) directly supervised by 17 staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing with small manipulative toys, napping and eating lunch. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-012-E-1
Description: Based on record review and interview, one staff member did not have required current certification in cardiopulmonary resuscitation (CPR). Evidence: The record for staff#2 did not contain documentation of a current CPR certification. According to the subsidy vendor agreement, staff is required to have a current CPR certification within 30 days of beginning employment.

Plan of Correction: We will have the employee trained as soon as a training is available.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review and interview, one staff member did not have required current first aid certification. Evidence: The record for staff#2 did not contain documentation of a current first aid certification. Description: Based on review of emergency response drills and staff interview, the center did not conduct lockdown drills annually. Evidence: The center did not conduct one lockdown drill annually.

Plan of Correction: We will practice a lockdown drill by the end of business day tomorrow.
