An unannounced monitoring inspection was conducted today with the director who was present with four additional staff and 42 children ( 3 of which were in clubs). The children were finishing snack ( celery, carrots, ranch dip and milk) and doing homework at the beginning of the inspection. After snack, they transitioned into zones. A sample size of six medications were inspected as well as five staff and eight children's files. Arrival time: 4: 35 pm Departure time: 5: 45 pm

Violations:
Standard #: 22VAC40-185-260-A
Description: Based on a review of the fire inspection report and an interview with the director, it was determined that the center did not have an annual fire inspection report. Evidence: The fire inspection report is dated 5/27/15.

Plan of Correction: I will continue to ask the school for an up-to-date annual report.
