Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Monday April 30, 2018 from 10:30am to 1:30pm. The children were observed listening to a story, practicing for an upcoming graduation, and coloring an octoagon. They were later observed eating lunch. Lunch consisted of fish sticks, corn, applesauce and milk. The lunch menu was posted. All classrooms, the playground and van were inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/23/2018 Last shelter-in-place drill: 04/23/2018 Total enrollment today: 44 Today, the following child to staff ratios were observed: Infants ?4:1 Toddlers ?5:1 Twos ? 16:2 Threes-10:2 Young Fours ?9:1 Pre-K ?10:1 School Age ? not present The violations from the previous inspections were checked for corrections. Description: Based on a review of children's records on 04/30/2018, the center did not ensure that if a child has had a physical examination prior to attendance, it shall be within two months prior to attendance for children six months of age and younger. EVIDENCE: The date of the physical completed prior to the Child #1's first day of attendance, which was 01/29/2018, is 10/24/2017. Child #1 was three months old.

Plan of Correction: Will make sure prior physicals aren't dated more than 2 months prior.

Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records on 4/30/2018, the center did not ensure that three of five records reviewed contained the required information. EVIDENCE: 1. The record of Child # 2 did not have a phone number documented for the place of employment listed for the child's father. 2. The record of Child # 5 did not have a phone number documented for the place of employment listed for the child's mother. 3. The record of Child #5 did not have an address for the second emergency contact listed. 4. The record of Child #1 did not have the previous child day care attended by the child documented.

Plan of Correction: Will go through all files and get missing information. Will do the same going forward.

Standard #: 22VAC40-185-280-B
Description: Based on observation on 04/30/3018, the center did not ensure that hazardous substances such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: A container of cleaning wipes were observed on top of the cubbies in the hallway leading to the three year old preschool class. The container was out of the reach of the children.

Plan of Correction: Corrected on site. Will remind staff cleaning supplies have to be locked up.

Standard #: 22VAC40-185-290-3
Description: Based on observation on 04/30/2018, the center did not ensure that in areas used by preschool children or younger electrical outlets shall have protective covers. EVIDENCE: An outlet in the science corner of the three's preschool room was uncovered.

Plan of Correction: Remind staff of daily room checks.
