63.2 Facilities and Programs..

Technical Assistance:
Discussed the Auvi - Q training with center director in case they receive any Auvi Q's in the future.

Comments:
An unannounced monitoring inspection was conducted from 2:10 pm-4:00pm. 73children were in care with 5staff members present. Children were observed during snack time, engaged in group activities and reading books. Handwashing and name to face accountability was observed. Children were observed going to their respective after school clubs. A selection of staff and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-270-A
Description: Based on observation, areas inside the center were not maintained in a safe and operable condition. Evidence: 1. A chair and couch in the second room in the center has large tears exposing the inside stuffing to the children. 2. Two cabinet doors are broken and removed from under the sink in the second room. The empty space has been covered by a poster board.

Plan of Correction: We have already submitted a work order for the cabinet door. We will follow up on the work order to get this fixed.

Standard #: 22VAC40-185-510-G
Description: Based on observation, prescription medication was not maintained in the original, labeled container. Evidence: 1. Child # 1 had emergency prescription medication on site. 2. The medication was dispensed for Child # 1's sibling and not form himself. 3. Child #1's name was handwritten on the prescription for the medication. The name for the child that the medication was dispensed for, was crossed off on the label.

Plan of Correction: We will talk to the parents today and return the mediation to obtain a new one.
