Licensing inspector discussed an expiring medication authorization, and service animals in a child care facility.
An unannounced monitoring inspection was conducted today from approximately 12:30pm-3pm. There were 125 children in care directly supervised by 25 staff. A sample of 10 children?s, 9 staff records and 10 medications were reviewed. The children were observed reading, napping and being dismissed. Two new staff members were hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-185-60-A Description: Based on record review, children's records were incomplete. Evidence: 1) Child A was missing the father's address, work phone numbers of the mother and father, and the addresses of 2 of 2 emergency contacts. 2) Child B was missing mother's employer, mother's employer phone number, addresses of 2 of 2 emergency contacts and fathers information. 3) Child C was missing 1 of 2 required emergency contacts, and the address of the existing emergency contact. 4) Child D was missing father's information and the addresses of 2 of 2emergency contacts. 5) Children E, F and G were missing father's information and the addresses of 2 of 2 emergency contacts. 6) Child H was missing the addresses of 2 of 2 emergency contacts. 7) Child I was missing the addresses of 2 of 2 emergency contacts and the work phone number of both parents. Plan of Correction: Student information file housed in front office binder is updated annually every August as needed. Coordinator has revised from to reflect required addresses. Child B mother employer should be removed as mother does not work and this should not apply.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: 1) Disinfectant wipes were found in an unlocked cabinet in the female gang bathroom. 2) Laundry detergent, and liquid absorbent powder was found in the laundry room where the door was propped open. Plan of Correction: Coordinator will obtain locks for laundry room and bathroom.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning and sanitizing materials were kept above food and eating utensils and were not stored in a physically separate from food. Evidence: 1) In Room #3, a bottle labeled sanitizer and disinfectant spray was found in a locked cabinet above plates and napkins. 2) In Room #4, disinfectant spray was found in a locked cabinet on the same level as food and above plates, cups and napkins. 3) In Room #5, a bottle labeled sanitizer and disinfectant spray was found in a locked cabinet on the same level as food and above plates, cups and napkins. 4) In Room #6, a bottle labeled sanitizer and disinfectant spray was found in a locked cabinet on the same level as food and above plates, cups and napkins. Plan of Correction: Coordinator sent email notification to all Head Start staff to resolve hazardous solutions location as to be separate from food and utensils. An observation from management is being conducted on April 13th and 16th 2018 to confirm item was resolved.
Standard #: 22VAC40-185-290-3 Description: Based on observation, an electrical outlet was not protected with a protective cover. Evidence: In Room #6, an outlet on the back wall near a shelf that held a radio was found without a protective cover. Plan of Correction: Coordinator sent email notification to all Head Start staff to resolve outlet cover. An observation from management is being conducted on April 13th and 16th 2018 to confirm item was resolved.
Standard #: 22VAC40-185-510-J Description: Based on observation, medication was not kept in a locked place. Evidence: The medication was located in the health administrators office in an unlocked cabinet. Plan of Correction: Health coordinator reported that medicines are locked at all times. Health coordinator was currently working on inventory and child files related to medication storage at the time of visit.
