An unannounced monitoring inspection was conducted on 4/30/18 from 3:00pm - 5:20pm. At the time of entrance there were 72 school age children in care with ten staff. Children were observed participating in various activities in the classrooms, doing homework, playing outside, and eating snack. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: 1. The record for staff #3 (date of hire (3/19/18) did not contain documentation of a negative tuberculosis screening. 2. Staff #6 (Program Director) reviewed the record for staff #3 and confirmed that there was not documentation of a negative tuberculosis screening.

Plan of Correction: The facility responded: Staff #3 had a TB screening at the beginning of the year for another Norfolk Academy program she was involved with. I have asked her to go back to that entity for a copy as the screening would be valid based on the DSS regs. I will forward asap. Sometimes getting high school students to have the same sense of urgency is a challenge.

Standard #: 22VAC40-185-550-D
Description: Based on a review of the emergency drill log and interview, it was determined that the facility did not ensure that they shall implement a monthly practice evacuation drill for the most likely to occur scenarios. Evidence: 1. The emergency drill log did not contain documentation to demonstrate that an emergency practice drill was completed during the month of February 2018. 2. Staff #6 (Program Director) confirmed that an emergency evacuation drill had not been completed during the month of February 2018.

Plan of Correction: The facility responded: We did a fire drill yesterday (5/1). I will place a reminder in my calendar to perform a fire drill on a monthly basis going forward.
