VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING

The licensing inspector conducted an unannounced complaint safety and health subsidy inspection regarding a serious injury in response to a complaint that was received by the licensing department on 04/12/18. The injury occurred at the center on 04/09/18. The inspection was initiated 04/17/18 from approximately 1:20 PM to 3:40 PM. Interviews with staff were completed, observations were made in the center and documentation was reviewed. Additional interviews were completed on 04/25/18. The preponderance of the evidence gathered during the investigation does support the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: VENDSUB-000-009-2 Description: Based on interviews, documentation review and observations, the center did not notify the department within two business days of a serious injury to a child while under the center's supervision. Evidence: During the 04/17/2018 inspection, the licensing inspector determined the following: 1. On 04/09/2018, staff stated the parents of Child A were notified about an incident that occurred in the Four Year Old classroom. The parent picked up Child A and took the child to the doctor on 04/09/18. 2. The center was notified on 04/10/18 that Child A was examined by a doctor on 04/09/18. 3. On 04/17/18, members of management confirmed to the licensing inspector that the department was not notified that Child A received medical attention as a result of the 04/09/18 incident. Plan of Correction: This was an oversight. We have previously reported and will ensure reporting is done within 2 days in the future. Reporting will be done by the director.
