Comments:
An unannounced monitoring inspection was conducted on May 4, 2018, between the hours of approximately 9:45 a.m. and 4:15 p.m. Sixty (60) children were enrolled and twenty-nine (29) children were in the care of five staff members, in four classrooms, during the on site inspection. School-age children arrived for care at 3:30 p.m. One additional staff member was present and assisted, as needed. Five children's records and five staff records were updated and/or reviewed. Two children's topical ointments, four children's medications, accompanying authorization forms and two medication administration logs were reviewed on this date. Safety equipment (first aid kit, flashlight, electrical outlet covers and radio), posted information (license, inspection documentation, weekly menu, emergency evacuation diagrams/plan, emergency telephone numbers and classroom schedules) and available documents (emergency evacuation drill/shelter-in-place drill records, attendance records, infant daily records, one local agency report, verification of insurance and injury reports) were observed/reviewed. The daily schedules accurately reflected the activities provided. The lunch served on this date included macaroni and cheese, mixed vegetables, peaches and milk. Observed staff/child interactions were positive and supportive of children's development. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Previously cited violations (inspection date - 12/14/2017) were reviewed for compliance; no repeat violations were cited. One exception to compliance was noted on the current violation notice. Description: Based on observation, measurements taken on the infant/toddler playground and discussion with staff on May 4, 2018, the program failed to ensure the resilient surfacing was adequate surrounding the exit end of the red slide and the exit ends of the blue slides. Evidence: 1. Measurements taken on the playground, on this date, determined that the surfacing at the exit ends of the blue slides measured 3 to 4 inches in depth. 2. Measurement taken on the playground and observation, on this date, determined that the exit end of the red slide lacked resilient surfacing.

Plan of Correction: The resilient surfacing will be raked and additional surfacing will be added, if needed.
