Comments:
Today an unannounced monitoring inspection was conducted, between 11:00am and 1:00pm. There were 6 classrooms observed with a total of 84 children and 15 staff. The children were observed coloring, painting, playing with sensory bags, reading and outside play. A complete inspection of the physical plant, 5 children and 5 staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of 5 children's files, the center did not obtain documentation of updated immunizations every 6 months for all enrolled children under the age of 2. Evidence: Child #3 and #4 (date of birth 10/21/16 and 6/26/17 respectively) did not have an updated immunization record on file.

Plan of Correction: I will contact the parents and get the updated immunization record.

Standard #: 22VAC40-185-240-D-1
Description: Based on medication review, the center did not include methods to prevent use of outdated medication. Evidence: Child #1 has a medication that expired 3/18.

Plan of Correction: I will contact mom and get new medication.

Standard #: 22VAC40-185-290-3
Description: Based on observation of classrooms not all electrical outlets have protective covers that are of a size that cannot be swallowed by children. Evidence: The Pre K and PS2 classroom's had electrical outlets that were not covered with protective covers.

Plan of Correction: I will put covers in. Corrected day of inspection.

Standard #: 22VAC40-185-370
Description: Based on conversation with staff, the center does not give documentation to parents regarding tummy time. Evidence: The center does not record any tummy time.

Plan of Correction: I will have the staff record the times of tummy time electronically daily.

Standard #: 22VAC40-185-430-B
Description: Based on classroom observation, equipment is accessible to children, that is not age and stage appropriate for children. Evidence: 1. In the PS2 classroom a box cutter was accessible to children in an unlocked drawer. 2. In the PS3 classroom adult scissors were accessible to children in an unlocked cabinet.

Plan of Correction: I will make sure the the box cutter is removed and the cabinet is locked. Corrected during inspection.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications, when an authorization for medication expires, the center did not notify the parent, that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. Evidence: Child #2 has a medication authorization on site that has an expiration date of 3/18/18.

Plan of Correction: I will return the medication to mom and get a new one.
