Comments:
An unannounced monitoring inspection was completed on 5/2/18. There were 30 children present with three staff supervising. Five children's records and three staff records were reviewed. Four emergency medications were observed. The inspector arrived at the program at 3:45 pm and departed at 6:00 pm. The children were observe during outside time and during departures.

Violations:
Standard #: 22VAC40-185-430-A
Description: Based on observation and interview, the center failed to ensure that equipment used by the children were of an appropriate size for the child using it. Evidence: 1. Two school-age children were observed swinging in two separate hard plastic, toddler-size, bucket swing seats on the playground. One child was squatting in the seat (sitting on her feet) and another was swinging with her legs in the leg holes of the bucket seat. Both children were six years old. 2.The manufacturer's instructions that were reviewed on line for a similar seat would be for use by children 6 months-1 year through three years. 3. The staff person who was within six feet of the two children swinging was asked by the inspector if these swings are allowed to be used by the children. The staff person replied, "Yes."

Plan of Correction: The staff will not allow any children to use these in the future.

Standard #: 22VAC40-185-510-G
Description: Based on observation, the center failed to ensure that two of four prescription medications had a parent's written authorization to administer the medications. Evidence: 1. There were two different medications (both albuterol inhalers) that were observed for children in care and did not have the parent's written authorization for administration. Both medications had written physician's authorization for administration.

Plan of Correction: The site director corrected one during the inspection and will get the second parent to sign the authorization at pick up.
