Technical Assistance:
Discussed with vendor: Tools/forms for updated subsidy requirements such as Proof of Identity and updated Emergency Response Plan.

An unannounced SHSI was conducted today from 3:40pm- 4:30 pm. There was 1 child ( 1 point)in care directly supervised by the vendor. The child was observed playing on the playground, watching TV, giving the inspector a tour of his play space and car activities and how he washes hands after potty routine. Description: Based upon information reviewed, required child information was incomplete. Evidence: Child A Child B Child C are missing mother's school address and emergency contact name. address and telephone number. Child D's information is missing parent work phone number and address, emergency contact name, address and telephone number, emergency authorization for Medical treatment signed by parent, sick policy.

Plan of Correction: Child just started and I text dad now to bring information. He has all papers to fill out.
