Standard #: 22VAC40-185-130-B Description: Based on record review, immunization records were not obtained every six months, for children under the age of two. Evidence: Child # 1's most recent immunization records were dated 8/2017. Updated immunization records were not obtained by the center. Plan of Correction: I will ask the parents to bring in updated records.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review, no staff on site had a current daily health observation training for children. Evidence: Staff # 3's daily health observation training expired on 5/23/17. Staff # 3's daily health observation training was not renewed. Plan of Correction: We have a training for daily health observation training scheduled for 5/25/18.
Standard #: 22VAC40-185-270-A Description: Based on observation, inside areas of the center were not maintained in a safe and operable condition. Evidence: There is a hole in the wall in the infant 2's classroom by the teacher closet. Plan of Correction: A work order for repairs will be placed.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and staff interview, center did not have a central registry finding within 30 days of employment for staff. Evidence: 1. Staff # 2 was hired on 10/3/17. 2. Staff # 2's record did not have the Virginia Central Registry search results. 3. Staff # 4 stated that when the paperwork was being filled out, staff # 2 put his home address instead of the center's address. 4. Staff # 4 stated that she called the agency last week to inquire about the form and left a message. Plan of Correction: I will re-submit the form.
Standard #: 63.2-1720.1-B-3 Description: Based on record review, center did not obtain the out of state central registry results for an employee that has resided outside of Virginia in the past 5 years. Evidence: 1. Staff # 1's record indicated that she has resided outside of Virginia, in two different states, in the past 5 years. 2. Center did not submit the out of state central registry search for one of the two states. Plan of Correction: I will submit the out of state central registry form for this staff person.
