Comments:
An unannounced monitoring visit and facility tour was conducted from 12:25 pm-2:05 pm. At the time of inspectors' arrival there were 93 toddler to preschool aged children in care with 7 staff members and 1 administrator. Children were observed participating in instructional time in the classroom, nap, transitioning between activities and toileting and hand washing. A sample of 9 children's records and 9 staff records were reviewed. Medications and injury reports were also reviewed. Areas of noncompliance were discussed with the center administration at the conclusion of this inspection and are detailed on the violation notice.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. There was four 1.5" areas of peeling drywall in the Toddler room. 2. There was a roll of plastic trash bags in the unlocked drawer in Staff #10's classroom. 3. The electrical outlet plate cover was separated from the wall leaving a 1/4" gap between the cover and the wall in Staff #10's classroom. 4. In Staff #10's classroom there was peeling paint on the changing table. 5. Outside near the walking labyrinth there was carpet/turf that was peeling up in 3 areas creating potential tripping hazards.
