BACKGROUND CHECKS for CHILD WELFARE AGENCIES

Comments:
An unannounced renewal inspection was completed on November 18, 2019, from approximately 9:25 am to 2:45 pm and on November 19, 2019, from approximately 10:20 am to 2:45 pm at Helping Other Peoples Enrichment (HOPE), doing business as Family Center of Hope, Inc. in Newport News, Virginia. The agency reports a total of 20 children in foster care and 28 approved provider homes.

During this inspection four provider home records, two foster child records and two personnel records were reviewed. Background checks for the corporate board officers were reviewed and are in compliance.

The Compliance Manager, Medical Records Supervisor and Executive Director, were available for the inspection and interview on 11/18/19. The Medical Records Supervisor was available for the exit interview which commenced on 11/19/18, at 2:20 pm. An Acknowledgment of Inspection form was signed and left at the agency.

There were four violations of the Standards for Child Placing Agencies. Please see the violation notice on the Department's web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-160-B-5
Description: Violation:
Based on a review of the record for Staff (S) S1 and interview with the medical records supervisor, the licensee failed to ensure the required information was documented in the personnel record.

Findings:
S1's hire date was 10/01/2002. The most recent ongoing training documented in the record for S1 is dated 12/8/17. During an interview with the medical records supervisor it was acknowledged that S1 has completed training during the current year however, the training was not documented in the record at the time of review.

Plan of Correction: S1 - During the exit interview, the Compliance Manager informed Licensure that S1 had previous trainings past 2017. S1's 2018-2019 trainings were in a folder locked in an office and the compliance manager did not have access to the folder at the time of the exit Interview. All trainings have been updated in S1's chart and have been scanned and emailed to Licensure along with this Plan of Correction.

Description: Violation:
Based on a review of the record for Foster Home (FH) FH2 and interview with the clinical manager, the licensee failed to issue a certificate of approval with the required elements.

Findings:
FH2 was initially approved by the licensee on 3/27/13. The re-evaluation dated 2/28/29 documents an approved capacity of 2 children. The placement log in the file documents that 3 children are currently placed in FH2.

During an interview with the clinical manager it was acknowledged that the record failed to document a new certificate of approval including the correct number of children recommended for placement into FH2.

Plan of Correction: FH2-A current certificate of approval was written on 11/20/19 and is in the FH chart to correspond with the addendum written on 9/18/19 changing the capacity of the home. All foster home charts will be audited to ensure the capacity of the home and the certificate of approval are accurate.

Description: Violation:
Based on a review of the record for Foster Home (FH) FH1 and interview with clinical manager, the licensee failed to ensure the approval certificate was documented in the record.

Findings:
FH1's initial approval was on 10/30/18. The record for FH1 documents a home study addendum dated 9/18/19 changing FH1's approval capacity. The record failed to document the current certificate of approval for FH1.

During an interview with the clinical manager, it was acknowledged that the current certificate of approval was not documented in the record at the time of review.

Plan of Correction: FH1 - A current certificate of approval was written on
11/20/19 and is in the FH chart to coincide with the addendum as it pertained to the change of capacity. All foster home charts will be audited to ensure the capacity of the home and the certificate of approval are accurate.

Standard #: 22VAC40-131-260-B-5
Description: Violation:
Based on a review of the record for Foster Child (FC) FC2 and interview with the medical records supervisor, the licensee failed to ensure the social history included required information.

Findings:
FC2 was placed with the licensee on 9/18/19. The social history documents that FC2 was previously placed with another licensed child placing agency. The social history failed to document the names of all previous caretakers.

During an interview with the medical records supervisor, it was acknowledged that the required information was omitted from the social history.

Plan of Correction: FC2 - The Social History states: "Name of facility and/or previous caregiver if known". The name of the providing agency was identified. The Social History was re-written as stated in the standards, to identify the child's previous placement history, including dates and names of previous caretakers If any. All staff will receive training on this standard and what is required going forward. As it pertains to FC2, the information has been obtained and an addendum to the Social History was added on 11/25/19.
