Technical Assistance:
1. Discussed clear and consistent documentation of biological family involvement. 2. Discussed submission requirements for applications to the department. 3. Discussed foster home reference checks; specifically ensuring that both parents are fully referenced on all referenced checks.

Comments:
An unannounced renewal inspection was conducted by two licensing representatives on June 11, 2019 from approximately 9:30 a.m-4:20 p. m. and June 12, 2019 from 8:45am-4:00p.m. at the Chester location. The licensee reports a total of 31 foster children in care at this location. The licensee reports a total of 42 active foster homes for this location. 4 personnel records were reviewed on 06/04/2019 at the Staples Mills location where personnel records are stored.

The following is a list of activities for this inspection:
4 foster care records were reviewed (includes 1 discharged record).
9 foster home records were reviewed (3 of the 9 foster home records were reviewed for background checks only).
A review of the Annual Outcomes Report for FY2018 was completed.

An entrance interview was conducted with Agency Representative (AR)1 and AR2. AR1 was available during the inspection. AR1, AR2, AR3 and AR4 were available for the exit interview which commenced at approximately 3:30 p. m. on June 12, 2019 at the Chester location. An acknowledgement form was signed and left with the licensee at this time. There were 2 citations for violations of Standards for Licensed Child-Placing Agencies.

Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-230-B
Description: Violation: Based on the review of the file for Foster Home (FH) 2 and based on an interview with AR1, at the time of inspection, there was no documentation of a visit or direct telephone contact with this approved home. Findings: 1. According to the approval letter and approval certificate both dated 2/25/2019, FH2 was approved on 02/25/2019, a 90 day monitoring visit or direct telephone contact was due on 05/25/2019. However, the record did not contain a visit to this approved home and did not contain documentation of a direct contact with this approved home. 2. Based on the interview with AR1, who reviewed the file, there was no documentation of a direct contact to this approved home and there was no documentation of a visit to this home.

Plan of Correction: The violation was unable to be corrected due to the monitoring visit not occurring within the 90days following the home approval.

TFC Documentation Specialist team to provide additional training with FSS (Family Services Specialist)/staff to address standard 22VAC40-131-(5)-230-B and documenting in the parent file/case note verification that a visit was conducted to the approved home.

Standard #: 22VAC40-131-250-L
Description: Violation: Based on the review of the file for Foster Child (FC) 1, the licensee failed to document a valid reason for not completing an interview with FC1 prior to placement. Based on the review of FC1?s file, the child has been in 3 prior placements. Findings: 1. FC1 file does not document a valid reason for not completing an interview prior to placement. 2. During the exit interview, AR1 and AR2 reported that FC1 was an emergency placement however, based on the review of the record, FC1 did not meet the standard definition of an emergency placement.

Plan of Correction: The violation was unable to be corrected due to the placement interview not occurring prior to or at the time of placement. For future (standard) placements the licensee shall conduct a pre-placement interview prior to deciding the appropriateness of placement.

TFC Documentation Specialist will provide statewide retraining with Intercept admissions/staff to ensure the understanding of LCPA regulation 22VAC40-131-(6)-250-L.
