Technical Assistance:
Technical assistance provided regarding Standard 250.N: summary of pre-placement visits.
Technical assistance provided regarding Standard 240.C: home study reports must contain adequate information and describe how the entire home accommodation the number of people residing in the home.

Comments:
An unannounced renewal inspection was conducted on February 18, 2022 from 9:00 am to 5:00 pm and February 18, 2022 from 9:00 am to 3:00 pm at the Chesterfield location. The licensee reports a total of forty-one (41) foster children in care at this location. The licensee reports a total of seventy (70) active foster homes for this location. There were four (4) newly hired staff since the last inspection.

The following is a list of activities for this inspection:

1. Four (4) foster child records were reviewed
2. Seven (7) complete provider home records were reviewed (including one re-evaluated home)
3. Five (5) additional provider home records (background checks only) were reviewed
4. Four (4) personnel records were reviewed
5. One (1) board officer record was reviewed
6. Applicable policy and procedures were reviewed
7. A physical plant inspection was completed

Staff were available for the exit interview which commenced at approximately 2:00 p. m. on February 18, 2022. There were 2 citations for violation of Standards for Licensed Child-Placing Agencies.

Upon receipt of the Violation Notice, the licensee is encouraged to develop a plan of correction to include the steps to correct non- compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Based on the review of the record for Foster Home (FH)-1 the licensee failed to conduct at least one of the face-to-face interview in the home of the applicant.

Findings:
1. The record documents a virtual face-to-face interview.
2. The record does not document a face-to-face interview in the home of the applicant.

Standard #: 22VAC40-131-290-C-8
Description: Based on the review of the record for Foster Child (FC)-3 and based on an interview with agency representatives (AR), the licensee failed to document a physical exam that contained evidence of freedom from communicable disease, including tuberculosis.

Findings:
1. The physical located in the record for FC-3, did not contain document of evidence of freedom from communicable disease, including tuberculosis.
2. During the preliminary findings/exit meeting, AR-2 reviewed the record and acknowledged the absence of a physical exam that documented evidence of freedom from communicable disease, including tuberculosis.
3. During the preliminary findings/exit meeting, AR-1 reported that during a recent audit, staff noticed the absence of evidence of freedom from communicable disease, including tuberculosis on the physical and responded by scheduling an appointment to obtain the regulated documentation.
