Technical Assistance:
Technical assistance discussion occurred regarding narrative documentation of phone calls and texts related to a foster child. Staff indicate a case narrative entry is completed for calls or texts that are logistical in nature such as conveying appointment information from one party to another and they have not required narrative content for such contacts. The Director of Foster Care and Independent Living agreed to assess this practice and ensure that any service delivery or progress information for foster children that may be discussed as a part of such logistical communications is being documented.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary
due to a state of emergency health pandemic declared by the Governor of Virginia.

An announced mandated renewal inspection was initiated on 8/11/2020 and concluded on 8/13/2020. The Director of Foster Care and Independent Living was contacted by email to initiate the inspection. The inspector emailed the Director of Foster Care and Independent Living a request for documentation required to complete the inspection. The agency reported 14 children in placement and nine (9) approved provider homes at the time of inspection. Two (2) foster child, three (3) foster home, and three (3) personnel records were reviewed during this inspection. Background checks for all Board Officers were reviewed.

The Director of Foster Care and Independent Living participated in the entrance conference and remained available during the inspection. The Director of Foster Care and Independent Living, Regional Director of Foster Care, and Caseworker participated in the exit interview on 8/13/2020. Two (2) violations of the Standards for Licensed Child Placing Agencies were cited.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review for Foster Child 1 (FC1), the agency failed to address all required elements in a written Quarterly Progress Report.

Findings:

The case record for FC1 included a Quarterly Progress Report for the first quarter of the child?s second placement year. The report stated the child ?Did not receive any medical treatment this quarter.? The record contained a physical examination report documenting the child received an annual physical on the fourth day of the reporting period. The findings were discussed during the exit interview and S1 acknowledged the findings.

Plan of Correction: Program Director will remind staff to include all medical documentation within the quarter when completing quarterly reports.

Description: Violation:

Based on record review for foster home 1 (FH1), the agency failed to maintain documentation in the file for FH1 a narrative account of the preparation of the family for each child placed in the home.

Findings:

The record for FH1 documented contained a blank Placement Narrative document addressing the preparation of the family for the placement of Foster Child 3 (FC3) in the home. The findings were discussed during the exit interview. Staff stated the information for Placement Narratives is located in foster child records and uploads to the Foster Home record. S1 located the Placement Narrative in the child?s file and provided to the Licensing Inspector. S1 acknowledged the findings.

Plan of Correction: Program staff S3 will ensure requisite documentation is in the FH1 file.


Program Director will remind staff to double check report submissions to ensure data entered is captured in the final form.
