Comments:

An unannounced monitoring inspection was completed on June 4, 2019 from 11:25 a.m. to 3:15 p.m. The licensee reports a total of thirteen children served in foster home placement or the agency's Independent Living Apartment program. During this inspection, two children?s records and two foster home records were reviewed. The Foster Care Supervisor was available for the inspection and interview and was present for the exit interview. The Director of Foster Care and Independent Living participated in the exit interview by phone. Inspection documents were completed and provided to the Regional Director. There was one citation for a violation of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on record review, the agency failed to place the physical exam for Foster Child 2 (FC2) in the child's case record within 30 days. Finding: A physical examination report for FC2 stating an examination date of 12/13/2018 was located in the child's file. The report includes a fax transmission date of 12/20/2018 to the agency's fax number. The agency date stamped the examination report as received on 4/4/2019. The agency's automated case file system indicates a submission date of 4/5/2019.
Plan of Correction:	The delay in date stamping was identified as an issue stemming from an incomplete form. Program Director will instruct staff to date stamp and upload all forms at time of original receipt and resubmit any amendments in a separate, date stamped record.
