Comments:
An unannounced monitoring inspection was conducted on June 24, 2019 from 9:45 AM to 2:30 PM. Commonwealth Catholic reports 28 approved provider homes and 32 children in placement. The agency has approved three provider homes since the last inspection. During this focused inspection to verify compliance, records for eight provider homes (background checks only) and one foster youth were reviewed. Interviews were conducted with three personnel.

An exit interview was conducted with the Program Manager at the conclusion of the inspection to review preliminary findings. An acknowledgement form was signed. There was one violation. Upon the receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-191-40-D-4-b
Description: Violation: Based on a review of the record for foster home 5 (FH5) and interview with staff 1 (S1), the agency failed to ensure background checks were completed within 30 days of the household member (foster child 1 - FC1) becoming 18 years old. Findings: 1. A sworn statement or affirmation, central registry search, and national criminal background check was not completed for foster child 1 (FC1) within 30 days of becoming 18 years old. 2. Staff 1 (S1) stated that the licensee has requested documents from the legal guardian which are needed to obtain identification for FC1. This identification is needed to process the background checks, and the needed documentation has not yet been received.

Plan of Correction: 1. CCC caseworker will contact the custodial agency to determine the progress of the processes in place to obtain legal identification for FC1 a minimum of every two weeks until the documentation is received. Upon receipt, an appointment with Fieldprint will be made by CCC to conduct the necessary clearance procedures. CCC Program Manager will oversee the progress of these contacts and the appointment with Fieldprint.
2. To prevent a recurrence of this violation, the placement of any child into CCC?s TFC program who is aged 17 years 0 days or older will be accompanied by immediate requests for legal identification needed from the custodial agency to complete the necessary background checks when the child turns 18 years of age. CCC caseworkers will offer any assistance needed to obtain these documents if they do not exist. Program Manager will keep ongoing oversight of the cases of all children aged 17 years, 0 days and older and will intervene directly with the custodial agency if documents are not obtained by the time the child reaches 17 years 6 months of age to create a plan to obtain the documents by the child?s 18th birthday to avoid a recurrence of the violation.
