Comments:

An unannounced renewal inspection was conducted on January 22, 2020 from 10:10 a.m. to 12:50 p.m. The licensee reports eight (8) youth currently placed in Independent Living. During this inspection one (1) youth record and two (2) personnel records were reviewed. Background checks for Board Officers were reviewed on 9/24/2019. 50% of personnel records for this site have been reviewed during the current license period. Interviews were conducted with the Executive Director and the Assistant Director regarding Organization and Administration. The Program Director, Executive Director, and Assistance Director participated in the entrance conference, remained available during the inspection, and participated in the exit interview. The exit interview was conducted from 12:10 p.m. to 12:50 p.m. An Acknowledgement of Inspection form was signed and left with the licensee at the end of the inspection. There were no citations for violations of the Standards for Child-Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.
