Comments:
An unannounced monitoring inspection was conducted on January 10, 2020 from 11:05 a.m. to 3:30 p.m. The agency reports six (6) children and youth placed in Independent Living Arrangements. During the inspection one (1) foster child record and three (3) personnel records were reviewed. Two Life Skill Coach Trainees (Caseworker Trainees), the Program Manager, and the Program Director (by videoconference) participated in the entrance conference. The Program Manager remained available during the inspection and participated in the exit interview. The Program Director joined the exit interview by videoconference.

The exit interview was conducted from 3:00 p.m. to 3:30 p.m. An acknowledgement form was completed and left with the agency at the end of the inspection.

There was one citation for a violation of the Standards for Licensed Child Placing Agencies. See the violation notice on the Department?s public web site for violations of the Standards.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation:

Based on record review, the agency failed to obtain a physical exam report addressing all required elements for Foster Child 1 (FC1).

Findings:

The record for FC1 contained a medical exam summary that did not address the child?s nutritional status, visual and auditory acuity, whether the child has any chronic conditions, and whether the child was free from communicable diseases. The report was not signed by the medical provider. The finding was discussed with agency staff during the exit interview. Staff reviewed the record and acknowledged the physical exam report did not address all required elements.

Plan of Correction: Executive Director will ensure all staff and program managers are fully trained on the requirements of the physical exam to include a signature from the medical provider for a foster child.
