Comments:

An unannounced monitoring inspection was conducted on April 25, 2019 from 8:46 AM to 3:05 PM. Holston Family Services reports a total of 23 provider homes and 45 foster children in placement. During this inspection, one personnel record, three provider home records and five foster child records were reviewed. Information reviewed for provider homes includes: home study requirements, home environment, provider pre-service training, background check information, provider home approval and re-evaluation, foster home agreements, annual training, monitoring of foster homes, home study addendum, and capacity/written justification. Information reviewed for foster child records includes: intake, acceptance, and placement information, social history, foster home agreements, medical, dental and psychiatric examinations and care, school enrollment, clothing and spending money, serious incident reports, service plans, quarterly progress summaries, discharge from care, contacts with the child, and case record requirements. Information reviewed for personnel records includes: annual training, annual evaluation, and personnel record requirements. An exit interview was conducted with the Foster Parent Coordinator to review preliminary inspection findings. An acknowledgement form was signed. There were no violations. The licensee has five (5) calendar days from receipt of the inspection documentation to sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days.
