An unannounced renewal inspection was completed on November 6, 2019 from 10:00 am to 2:00 pm at Children's Home Society of Minnesota (CHSM) in Springfield, Virginia. CHSM provides home study and post-placement supervision services for domestic and international adoptions. CHSM also provides parental placement services. There are 11 homes approved and waiting for adoptive placements. No substantive program changes were reported. During this inspection, five adoptive home records, and three personnel records were reviewed. One child's file was also reviewed. In compliance with licensing requirements for renewal application, all documentation for corporate board officers and agents of the corporation were reviewed, including required background checks. The agency's Chief Administrator was available for the inspection and the exit interview at the end of the day. There were two citations for violations of the Standards for Child Placing Agencies. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) business days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of the adoptive home record (AH-1) and an interview with the agency representative, the agency failed to include required documentation in the home study. Evidence: 1) The home study for AH-1 did not include a confidentiality statement signed and dated by the prospective adoptive parents. 2) The agency representative reviewed the file and agreed that it did not contain a confidentiality statement signed and dated by the prospective adoptive parents as required.

Description:	Violation: Based on a review of the personnel record for (P-1) and an interview with the agency representative, the agency failed to obtain a criminal history record check and a CPS check for the employee within 30 days of the start of his employment with the agency. Evidence: 1) P-1 began employment with the agency more than 30 days prior to this inspection. 2) The criminal history record check was received 50 days after the start of his employment. 3) There was no CPS check in the file. 4) The agency representative reviewed the file and agreed that the criminal record check was outside the 30 day timeframe and that the CPS check was not in the record.
