Comments:
An unannounced monitoring inspection took place on November 26, 2019, from 10:00 am to 2:00 pm at Adoption Home Studies by Loving Families, located at 2106 Mountain Road, Lovettsville, Virginia, 20180. The agency reports eight families approved and waiting for placement. Five families are receiving post placement supervision services. The Executive Director was available for the inspection and present for the exit interview at the end of the inspection.

During this inspection, three adoption records were reviewed. One child file was reviewed as post placement supervision services were being provided to the child and family. In compliance with licensing requirements for renewal application, all documentation for corporate board officers/personnel were reviewed, including required background checks. There were two citations for violations of the Standards for Licensed-Child-Placing Agencies. One was a key health and safety standard. An acknowledgement of Inspection form was signed on this date.

Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Violation: Based on a review of the adoptive home records (AH-2 and AH-3) and an interview with the agency representative, the agency failed to obtain signed authorizations to contact the previous child-placing agencies that approved both adoptive families.

Evidence:
1) AH-2 and AH-3 were both previously approved by other child-placing agencies. There were no signed consent to exchange information forms in the adoptive family files allowing the agency to communicate with the original agencies.
2) The agency representative reviewed the files at the time of the inspection and acknowledged that signed authorizations were not obtained as required.

Plan of Correction: The Executive Director will ensure all applicants sign a release to allow her to communicate with previous child-placing agencies and to obtain references when necessary.

Description: Violation: Based on a review of the adoptive home record (AH-1) and an interview with the agency representative, the agency failed to verify that the adoptive parents have in-force vehicle liability insurance.

Evidence:
1) There was no documentation in the adoptive family file of AH-1 of vehicle insurance.
2) The agency representative reviewed the file at the time of the inspection and acknowledged that the family's car insurance was not verified.

Plan of Correction: The Executive Director will verify car insurance for all adoptive home applicants as required.
