Technical Assistance:
Technical Assistance was provided on procedures to add additional licensed locations and definition of child-placing activities.

Comments:
An unannounced monitoring inspection was conducted on January 22, 2020 from 2:25 PM ? 4: 40 PM and on January 23, 2020 from 9:20 AM - PM 3:05 PM. Alliance Human Services reports a total of 55 provider homes and 42 foster children in placement. During this inspection, seven provider home records, five foster child records, and one personnel record were reviewed. Three personnel were interviewed during the inspection. An exit interview was conducted at the conclusion of the inspection with the Executive Director and Program Director to review preliminary inspection findings. An acknowledgement form was signed. There were three violations.

Upon the receipt of the violation notice, the licensee should identify the necessary corrective action and develop a plan of correction for each violation. The plan of correction should include the following: The steps to correct noncompliance with the standard(s); measures to prevent re-occurrence of noncompliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section titled Plan of Correction, sign each page of the documentation and return it to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection documentation are subject to public disclosure and will be posted on the VDSS web site within 5 days, regardless of whether the Plan of Correction is completed.

Violations:
Standard #: 22VAC40-131-180-J
Description: Violation:
Based on the review of the files for provider homes (PH) and interview with personnel (S), the agency failed to address an assessment required by 22VAC40-131-180.J.2.e.

Evidence:
1. The narrative section of the home studies for PH2, PH3, PH4, PH5, and PH6 did not document an assessment that the applicant(s) has/have the financial resources to provide for current and on-going household needs and maintenance of the family (22VAC40-131-180-J.2.e.9).
2. When discussed during the exit interview, S1 acknowledged that this assessment was not included specifically in the home study although an overall assessment of the home was provided in the recommendation.

Plan of Correction: Addendums will be completed on the home studies that did not document the assessment that the applicant has the financial resources to provide for the current and on-going household needs and maintenance of he family.

A training will be done to ensure that the Program Recruiters are aware of the requirement to include the assessment of the financial resources in the home study.

The Program Director and Executive Director will review all home studies and ensure that the assessment is documented.

Standard #: 22VAC40-131-260-B
Description: Violation:
Based on a review of the record for a foster child (FC3) and an interview with personnel (S1) the agency did not include required elements in the social history.

Evidence:
1. The social history of FC3 did not address: 260.B.8 Education and occupation of the child's parents, siblings, aunts, uncles, and grandparent; 260.B.9 Medical and psychiatric history of the child's parents, siblings, aunts, uncles, and grandparents.
2. During the exit interview, S1 acknowledged that this information was not captured in the social history for FC3.

Plan of Correction: A training will be conducted with the Case Manager to ensure that the staff are aware of the requirement to address the education, occupation, and medical and psychiatric history of the child's parents, siblings, aunts, uncles, and grandparents in the social history.

Standard #: 22VAC40-131-290-L-1
Description: Violation:
Based on a review of the record for a provider home (PH5) and interview with personnel (S1), the agency failed to obtain an examination for the applicant from PH5 which included written examiner comments adressing PH5's mental condition in relation to her ability to take care of a child.

Findings:
1. The applicant from PH5 submitted a medical examination as part of the home study process.
2. The section entitled 'comments on emotional stability/maturity' was not addressed as indicated by the physician writing "N/A".
3. The home study narrative for PH5 contained the following statement regarding the applicant in the section 'comments on emotional stability/maturity': "there are no comments listed on her emotional maturity and stability".
4. When discussed during the exit interview, S1 acknowledged the above facts.

Plan of Correction: A training will be conducted with the Program Recruiters on the requirement to address the mental condition of the applicant in relation to their ability to take care of a child.

The Program Director and Executive Director will review all caregiver reviews and home studies to ensure that this standard is met.
