Comments:

An unannounced monitoring inspection took place on February 4, 2020 from 10:00 am to 2:00 pm at the offices of Adoptions Simply Wonderful, Inc., located at 2936 Brierwood Road, Petersburg, Virginia 23805. Presently, there are twelve approved provider homes. There are no children in post-placement supervision by this agency. The Executive Director was available for the inspection and present for the exit interview at the end of the day. During this inspection, three adoptive home records were reviewed in their entirety. Background checks were reviewed in a fourth provider home record approved since the last inspection. There was one citation for a violation of the Standards for Child Placing Agencies. It was a key health and safety standard. An Acknowledgement of Inspection form was signed on this date. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on a review of the record and an interview with an agency representative, the agency accepted background checks dated more than 90 days prior to the approval of a provider home (AH-2). Evidence: 1) The record for AH-2 contained criminal history record results for both providers that were dated more than 90 days prior to their approval as resource providers. 2) The agency representative reviewed the file at the time of the inspection an acknowledged that this was an oversight.
Plan of Correction:	The Executive Director will ensure moving forward that all required background checks are dated within 90 days of approval or will contact the division to apply for a variance request.
