Licensing specialist discussed job descriptions for interns and written plan for interns with the executive director.

Comments:

An unannounced renewal inspection was conducted on 02/11/2020 from approximately 12:20 p.m. to 5:00 p.m. The agency reports five children in care and six approved provider homes. During the inspection one foster child record, one provider home record and 6 personnel records were reviewed. Background checks for board officer was reviewed. The 2019 program effectiveness and evaluation plan was reviewed. An exit interview was conducted on 02/11/2020 at 4:00 p.m. with the executive director to discuss the inspection findings and an acknowledgment of inspection form was left at the end of the inspection. There were 3 violations of the Standards for Licensed Child-Placing Agencies. Please see the violation notice on the Department?s website for violation of the Standard. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Based on review of the records for Staff (S) S1,S2 and interview with executive director, the licensee failed to ensure a copy of S1's educational degree was in the record and an annual performance evaluation in the record for S2. Findings: S1 was hired on 8/19/19 as chief executive officer. The record for S1 failed to document a copy of the required educational degree. S2 was hired in 9/2009 and at the time of review, the most recent performance evaluation in the record for S2 was dated 5/9/18.

Description:	Based on review of the record for Foster Home (FH) FH1 and interview with the executive director, the licensee failed to ensure a confidentiality statement was signed and that FH1's marital status was verified. Findings: FH1 was approved by the licensee on 1/29/2020. The record failed to document a signed confidentiality statement and verification of FH1's marriage. The record documented a divorce decree for Applicant (A) A1, however there was no verification of current marital status in the record.

Description:	Based on a review of the record for Foster Child (FC) FC1, the licensee failed to list medical needs for FC1 during the review period. Findings: FC1's record documents contact notes that medical treatment was received on 12/15/19 and that the child was prescribed medication. The written quarterly progress review covering 11/3/19 to 2/2/2020 fails to document the medical treatment received and the medication prescribed. During an interview with the executive director, it was acknowledged that the information was omitted from the progress review.
