Comments:
An unannounced renewal inspection took place on March 10, 2020 from 1:05 pm to 2:10 pm at Catholic Charities of Eastern Virginia (CCEVA), located in Virginia Beach, Virginia. The human resources files for all of the Virginia licensed offices are maintained at this office. There are no birth parent/child or provider home case records kept at this location, although the office is available to conduct any needed child-placing activities. The Director of Adoption of Child Welfare was available for the inspection and present for the exit interview, which commenced on 3/10/20 at approximately 2:00 pm.

The background checks for the corporate board officers were reviewed during an inspection at the Norfolk office of CCEVA on 3/9/2020 and are in compliance. During this inspection 5 personnel records were reviewed. The agency's performance quality improvement plan was discussed with the director of adoption and child welfare.

There were two citations for violation of the Standards for Child Placing Agencies. Please see the violation notice on the Department's website for violations of the Standards. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected.

The licensee has five (5) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Standard #: 22VAC40-131-160-B
Description: Violation:
Based on a review of the record for Personnel (P) P1 and interview with the director of adoption and child welfare, the licensee failed to ensure P1's record documented a job description and an educational degree.

Findings:
P1 was hired in October 2019. At the time of review the record did not document a job description or an educational degree. During an interview with the director of adoption and child welfare, it was acknowledged that the information was not in the record at the time of review.

Plan of Correction: The Director of Adoption & Child Welfare will ensure that the HR Manager adds the two missing documents (signed job description & copy of educational degree) to employees file.

The Executive Director of Child Welfare me with the HR Manager on March 12, 2020 to ensure that the HR Manager has a clear understanding of all documents that are required to be in an employee?s file. A full review of all HR files will take place in April 2020 and after that random internal audits of files will take place to ensure compliance.

Description: Violation:
Based on a review of the record for Personnel (P) P1 and interview with the director of adoption and child welfare, the licensee failed to ensure a search of the central registry was completed before 30 days of employment ended.

Findings:
P1 was hired in October 2019. The record documents a central registry search dated 12/9/2019 as a result of the request not being sent in a timely manner. During an interview with the director of adoption and child welfare, it was acknowledged that the central registry was out of compliance.

Plan of Correction: The Director of Adoption & Child Welfare will send to the HR Manager the Background Check Guideline Policy sheet that was created in July 2018 again for review.

The Executive Director of Child Welfare met with the HR Manager on 3/12/2020 to ensure that the HR Manager has a full understanding of the requirements and that these are followed implicitly moving forward.
