Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 5/28/20. The inspection was concluded on 6/09/20. The District Supervisor was contacted by telephone to initiate the inspection. The agency reports 10 children in care and 21 approved provider homes. The inspector emailed the District Supervisor a list of items required to complete the inspection.

The inspector reviewed documents from 3 provider home records and 1 birth parent/child's record submitted by the agency. Background checks for 9 providers approved since the last inspection were also reviewed. 4 personnel records were reviewed. Background checks for 2 corporate officers were reviewed in compliance with regulations regarding license renewals.

An exit interview was conducted on 6/09/20 with the District Supervisor at 3:30 pm.

Information gathered during the inspection determined non-compliances with applicable standards or law, and the violation was documented on the violation notice issued to the agency.

Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Standard #: 22VAC40-131-180-J-2
Description: Violation: Based on a review of the provider record for adoptive home 2 (AH-2) and an interview with an agency representative, the agency failed to assess that the family had financial resources to provide for current and ongoing household needs and maintenance of the family.

Evidence:
1) A financial statement contained in the record documented a net monthly income of $4,895.00 and monthly expenses of approximately $2,559.00. This leaves a monthly surplus of $2,336.00
2) Paystubs contained in the record document a net monthly income of $4,382.46.
3) The homestudy contained in the record documents monthly expenses to be $1,500.00 and states that the family has a monthly surplus of $3,400.00.
4) The figures provided from the financial statement, the paystubs, and the homestudy do not match.
5) During the exit interview the agency representative acknowledged that the figures don't add up and that the family's income and expenses should have been more accurately assessed.
