Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An announced monitoring inspection was initiated on 01/26/2021 and concluded on 02/16/20201. The executive director was contacted by telephone to initiate the inspection. The agency reported 18 children in care and 22 approved provider homes when the inspection was initiated. The inspector emailed the executive director a list of items required to complete the inspection. The executive director and general compliance manager were available to answer questions throughout the inspection. The inspector reviewed 2 provider home records, 2 children records and 5 staff records. Background checks for 10 additional provider homes were reviewed and two employees were interviewed. The agency submitted Covid-19 policies and procedures. An exit interview was conducted by telephone on 02/16/21 from approximately 4:00 pm-5:30 pm with the executive director and the general compliance manager. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. There were 3 citations for violation of the Standards for Child-Placing Agencies. Upon receipt of the Violation Notice, the licensee should develop a plan of correction to include the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which noncompliance will be corrected. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of Correction", sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public web site within five business days, regardless of whether or not the Plan of Correction is completed.

Description:	Violation: Based on review of background checks for Provider Homes (PH) PH2, PH3, PH4, PH5, PH6, PH7, FH8, PH9, PH10, PH11, and PH12 the licensee failed to document date received on the central registry findings and/or criminal record checks. Findings: Criminal history record and/or central registry findings for adults in PH2, PH3, PH4, PH5,PH6,PH7, PH8, PH9, PH10, PH11, and PH12 fails to document date received.

Description:	Violation: Based on review of the medical examination for Foster Child (FC) FC1, the licensee failed to ensure the annual medical examination addressed FC1?s auditory acuity and allergies. Findings: FC1 was placed with the licensee 10/23/20. FC1?s annual examination dated 1/28/20 fails to address FC1?s auditory acuity and allergies.

Description:	Based on review of the background checks accepted for Provider Home (PH) PH7, the licensee failed to ensure the required background check was dated no more than 90 days prior to approval. Findings: PH7 is a two-parent home. The central registry results for the second provider in PH7 is dated 9/1/20. PH7 was approved on 1/18/21. 9/1/20 is more than 90 days prior to the date of approval.
