Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 04/05/21. The inspection was concluded on 04/15/21. The Chief Administrator was contacted by email to initiate the inspection. The agency reports 17 families approved and waiting. The inspector emailed the Chief Administrator a list of items required to complete the inspection. The inspector reviewed documents from 3 provider home records and 1 personnel record submitted by the agency. An exit interview was conducted on 4/15/21 with the Chief Administrator. Information gathered during the inspection determined non-compliances with applicable standards or law, and violations were documented on the violation notice issued to the agency. Upon receipt of the violation notice, the licensee must develop a plan of correction for each violation. The plan of correction must include the following: the steps to correct non-compliance with the standard(s); measures to prevent re-occurrence of non-compliance; person(s) responsible for implementation and monitoring of preventative measure(s); and date by which non-compliance will be corrected. The licensee has ten (10) calendar days from receipt of the inspection documentation to complete the section entitled "Plan of Correction", sign each page of the Plan of Correction and return it to the Licensing Office. The licensee retains a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the VDSS web site within five (5) days, regardless of whether or not the Plan of Correction has been completed.

Description:	Violation: Based on a review of background checks for an adoptive family (AH-1) and an interview with an agency representative (AR-1), the agency accepted criminal history record reports for the adoptive parents that were dated more than 90 daysprior to the approval of the adoptive home. Evidence: 1) AH-1 is an approved adoptive home. Background checks for both adults in the home are required prior to approving the adoptive home. 2) The criminal history record reports for both adoptive parents (AH-1) were dated more than 90 days prior to the date the adoptive home was approved. 3) In an interview with AR-1 on 4/15/21, AR-1 acknowledged that the criminal history record checks for AH-1 were dated more than 90 days prior to the date the adoptive home was approved.
