63.2 (1) General Provisions

Technical Assistance:
22VAC40-131-330.G.3: A discussion regarding the format of documentation capturing training and guidance to foster parents.

Comments:
A renewal inspection was initiated on August 3, 2021 and concluded on August 6, 2021. The Program Director was contacted by telephone to initiate the inspection. The Program Director reported that the current census was 31 foster children, 35 active foster home providers and 2 newly hired staff since the last inspection. The inspector emailed the Program Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 3 foster children records, 5 provider home records, 2 employee records, and board officer records submitted by the facility to ensure documentation was complete. An exit interview was conducted with agency representatives on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance(s) with applicable standards or laws, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-131-330-G
Description: Based on the review of the records presented for Foster Child (FC)-3 and based on an interview with Agency Representatives (AR), the licensee failed to document a face to face contact in the home for the month of June 2021 and a private interview with the child in the month of May 2021.

Findings:
1.Based on the review of the records for FC-3, the contacts in the month of June 2021 were documented outside the home with none documented within the home.
2.Based on the review of the records for FC-3, there was no documentation of a private interview with the child in the month of May 2021.
3.During the exit interview, AR-1 and AR-2 reviewed the records and acknowledged the absence of documentation of a private interview in the month of May 2021. AR-1 and AR-2 acknowledged the absence of a documented home visit in the month of June 2021.

Standard #: 22VAC40-131-340-E-3
Description: Based on the review of the records presented for Foster Child (FC)-1 and based on an interview with Agency Representatives (AR), the licensee failed to develop an individualized service plan with goals and objectives to meet the child?s needs that include specific independent living services to be provided to assist the child in meeting his goals for a youth 14 years of age or older.

Findings:
1.Based on the review of the individualized service plan for FC1, there were no independent living goals or objectives listed on the plan.
2.Based on the review of the individualized service plan for FC1, the plan did not list goals and objectives to meet the child?s needs that include specific independent living services.
3.During the exit interview AR-1 confirmed the youth?s age of 16 and acknowledged the absence of an individualized service plan that documented goals and objectives to meet the child?s needs that include specific independent living services.
