Comments:

A monitoring inspection was initiated on8/11/21 and concluded on 8/16/21. The general compliance manager was contacted by telephone to initiate the inspection. The general compliance manager, reported that the current census was 17 children in care and 34 approved provider homes. The inspector emailed the general compliance manager a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 2 children records, 11 provider home records and 10 staff records. One employee was interviewed during the inspection. An exit interview was conducted with the general compliance manager on the 8/16/21 from approximately 11:00 am - 11:45 am, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on review of contact notes provided for FC1, the licensee failed to provide training and guidance to the foster parent. Findings: Contact notes for May-July 2021 for FC1 were reviewed. The contacts for May-July 2021 did not document training and guidance provided to FC1?s foster parent.

Description:	Violation: Based on review of the individualized service plan for Foster Child (FC) FC1, the licensee failed to include criteria for achievement of goals and objectives as required by Standard 22VAC40-131-340-E-3.b. Findings: FC1 was placed on 4/10/21 for treatment foster care services. The individualized service plan completed by the licensee is dated 5/2021. The May 2021 service plan fails to document criteria for achievement for all goals and objectives identified as required by Standard 22VAC40-131-340-E-3.b.

Description:	Violation: Based on review of the FC1?s individualized service plan, the licensee failed to include a description of programs and services to assist the youth in his transition from foster care to independent living. Findings: FC1 was placed with the licensee on 4/10/21 as a treatment foster care placement. FC1 is 17 years of age. The individualized service plan for FC1 dated 5/20/21 fails to document a description of programs and services to assist FC1, with his transition from foster care to independent living.

Description:	Violation: Based on review of background checks, application, sworn disclosure and interview with agency representative (AR) AR1, the licensee failed to ensure a search of the central registry for abuse and neglect maintained by any other state in which the prospective foster parents resided within the last five years was obtained prior to approval. Findings: The provider home (PH) record for PH6 contains a provider application received from the foster parents (P) P1 and P2. The provider application, dated 10/19/2020, documents that P1 and P2 have resided in state (S) S2 within the last 5 years. The record for PH6 does not contain the results of a search of the child abuse and neglect registry maintained by S2 for P1 and P2. During the exit interview AR1 acknowledged that out of state child abuse and neglect registry checks for P1 and P2 in PH6 were not completed prior to PH6?s approval.

Description:	Violation: Based on review of face sheet, sworn disclosure, background checks, and interview with agency representative (AR) AR1, the licensee failed to obtain results of a search of the child abuse and neglect registry maintained by other state where the individual resided in the last five years. Findings: PH8 was approved on 4/12/21. The face sheet for PH8 documents other household member (OHM) OHM1 resides in PH8.The sworn disclosure dated 3/30/21, for OHM1 documents that OHM1 resided in State (S) S1 within the last 5 years. No out of state central registry check results were provided for OHM1. During the exit interview, AR1 acknowledged the results for OHM1?s out of state central registry check have not been obtained by the licensee.
