Comments:

A monitoring inspection was initiated on 9/27/2021 and concluded on 09/30/2021. The Virginia District Supervisor was contacted by telephone to initiate the inspection. The Director of Corporate Social Services reported that the agency currently has 47 families approved and waiting for placement. The inspector emailed the Director of Corporate Social Services a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed five (5) adoptive home records and six (6) personnel records submitted by the agency to ensure documentation was complete. An exit interview was conducted with the Director of Corporate Social Services, the Chief Operating Officer, and the Human Resources Director on 09/30/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation was documented on the violation notice issued to the agency.

Description:	Violation: Based on a review of documents submitted by the agency for an employee (P-3) and an interview with an agency representative (AR-1), the agency accepted a criminal history record report and a central registry search for an employee that were dated more than 90 days prior to the date of employment. Evidence: 1) The criminal history record check and the central registry search for P-3 were more than 90 days old when the employee began working at the agency. 2) In an interview with the agency representative on 9/30/21, AR-1 acknowledged that the criminal history record check and the central registry search for P-3 were more than 90 days old when P-3 began employment with the agency.
