Comments:

A monitoring inspection was initiated on 10/08/2021 and concluded on 10/12/2021. The Chief Administrator was contacted by telephone to initiate the inspection. The Chief Administrator reported that the agency currently has 13 families approved and waiting for placement. The inspector emailed the Chief Administrator a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two (2) adoptive home records and two (2) personnel records submitted by the agency to ensure documentation was complete. Background checks for four (4) adoptive families approved since the last inspection were also reviewed. An exit interview was conducted with the Chief Administrator on 10/12/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation was documented on the violation notice issued to the agency.

Description:	Violation: Based on a review of the background checks submitted by the agency for two staff members (P-1, and P-2) and an interview with an agency representative (AR-1), the agency failed to obtain new central registry findings and new criminal history record check reports before three years since the dates of the last central registry findings and criminal history record check reports for each employee. Evidence: 1) The criminal history record check reports provided by the agency for review for P-1 and P-2 were dated for February of 2018. 2) The central registry findings provided by the agency for review for P-1 and P-2 were dated for February and April of 2018. 3) In an exit interview on 10/12/21, AR-1 acknowledged that the background checks for each employee were outdated.
