Comments:
A renewal inspection was initiated on 10/18/2021 and concluded on 10/26/2021. The Executive Director was contacted by telephone to initiate the inspection. The Executive Director reported that the agency currently has 6 families approved and waiting for placement. The inspector emailed the Executive Director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed three (3) adoptive home records and one (1) personnel record submitted by the agency to ensure documentation was complete. There were no changes to any polices or procedures reported. Background checks for the Corporate Officers were also reviewed. An exit interview was conducted with Executive Director on 10/26/2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law, and the violation was documented on the violation notice issued to the agency.

Description: Violation:
Based on a review of the background checks submitted by the agency for a corporate officer (CO-2) and an interview with an agency representative (AR-1), the agency failed to obtain a new central registry finding and a new criminal history record check report before three years since the dates of the last central registry finding and criminal history record check report for the corporate officer.

Evidence:
1) The criminal history record check reports provided by the agency for review for CO-2 was dated for August of 2017.
2) The central registry finding provided by the agency for review for CO-2 was dated for for August of 2017.
3) In an exit interview on 10/26/21, AR-1 acknowledged that the background checks for the corporate officer (CO-2) were outdated.
