Licensing Inspector discussed 22VAC40-131-230-I with the compliance manager.

Comments:

A non-mandated focused monitoring inspection was initiated on 11/04/2021 and concluded on 11/09/2021. This announced focused inspection was conducted to determine compliance with the intensive plan of correction submitted. The general compliance manager was contacted by telephone to initiate the inspection. The general compliance manager, reported that the current census was 14 children in care and 28 approved provider homes. The inspector emailed the general compliance manager a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed 1 provider home record. The licensee's provider home background check tracking spreadsheet was reviewed. The general compliance manager was interviewed during the inspection. An exit interview was conducted with the general compliance manager on the 11/09/21 from approximately 2:50 pm to 3:00 pm , where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Description:	Violation: Based on review of the face sheet for Provider Home (PH) PH1, the licensee failed to ensure the face sheet listed the date of birth for all household members. Findings: The face sheet for PH1 documents two biological children reside in the home. The face sheet fails to document the dates of birth for the biological children.
